KHPT is seeking applications for the position of Community Facilitators to improve health and wellbeing of communities in India. There are 3 vacancies for this role, based in Jigani, Bangalore.

Eligibility Criteria: Qualifications and Skills
SSLC, PU or any graduate in any relevant field is desirable.
Minimum of 5 years of professional experience in the social development sector.
Willingness and ability to travel extensively in the field areas of Urban PHC in Bengaluru is essential.
Good spoken and written skills in English and the local language are desirable.
Good knowledge of Excel and MS-Office is desirable.
Prior experience of working in a CPHC-related project is an added advantage.
Roles and Responsibilities for Community Facilitators
Develop the micro plan of the area by involving FLWs (ASHA, AWW & ANM) and other key stakeholders.
Should be located in and around Jigani UPHC for regular field-level engagement.
Develop the micro plan of the intervention area in coordination with FLWs (ASHA, AWW, ANM) and key community stakeholders.
Conduct enumeration of vulnerable populations in the Jigani UPHC area and track them through the NCD continuum of care.
Conduct home-based Communication sessions with identified vulnerable beneficiaries.
Mobilize beneficiaries for group sessions such as Peer Support Group Meetings.
Mobilize community members for health camps.
Ensure timely identification and referral of beneficiaries showing complications/danger signs to appropriate facilities and track their status.
Participate in monthly PHC-level ASHA meetings, AWW meetings, and SHC-level reviews to discuss progress and challenges.
Work with Mahila Arogya Samiti (MAS) and UHSNCs and ward-level meetings to build awareness on community participation and seek support for complex cases.
Build and maintain rapport with local functionaries of DoHFW and other relevant departments.
Participate and contribute in project review and planning meetings.
Reporting Structure
The Community Facilitator will report to the Project Officer-HCL or person designated by him/her.
Remuneration Details
Compensation will adhere to internal policies and market standards, determined by qualifications, relevant experience, budget availability, internal parity, and interview performance.
